Chapel of St. Panny Marie nad Javor

If you go from Dědov to Skály (see https://www.turistika.cz/turisticke-trasy/detail/skaly-z-police-nad-metuji-pres-ostas), turn left at the end of the village and pass an unattractive agricultural yard, you ascend to the edge of the forest.
